Create Your Own Case Consultation Form

I’ve got a challenge/request for you:

Can you create your own case consultation form?

Moving forward, for all case presentations, I’d like you to fill out your form in advance and bring it to share in our sessions. This could be one you’ve got from a workbook, a textbook, from a basic google search, or one that you’ve completely created on your own. This website has some good ones.

Why?

• I’ve found this makes for a much more efficient case presentation.

• One goal I have is for each of you to have mastery of a few modalities by the time we’re done working together; to have them as a roadmap in your back pocket from which you can wander and explore, and return to when needed. I think case consultations in this way will help with that.

• This will improve your documentation skills.
